Our goal at Annapolis Family Medicine is to help our patients achieve total body wellness and health using a multidisciplinary approach to health care. In keeping with our goal of providing the best possible care and up to date therapies to our patients, Annapolis Family Medicine will be offering evaluations for medicinal cannabis use for patients with specific medical conditions.
Medicinal cannabis was legalized by the state of Maryland in 2012, and is recognized in the medical community as an alternative therapy for certain conditions when traditional medical treatments have failed. - Visit the following webpage to register with the Maryland Medical Cannabis Commission: www.mmcc.maryland.gov. You must register prior to your visit,or face a delay in receiving your card.
- You must bring with you at least one years worth of medical records which detail your qualifying medical conditions and treatments that have failed. Qualifying medical conditions include: cachexia, anorexia, wasting syndrome, severe pain, severe nausea, seizures, severe or persistent muscle spasms, glaucoma, post-traumatic stress disorder, and chronic pain. If you are not a current patient of Annapolis Family Medicine, please fill out this form and send it to your current pain management or primary care provider. They will forward the appropriate records to us directly at no charge.
- You must sign our patient contract. Please read, review, and sign prior to your appointments. This contract includes consent for treatment.
The cost for your initial evaluation and your state mandated annual follow-ups will be $190. This will need to be paid in full by cash or credit at the time of your visit. Annapolis Family Medicine is mandating additional follow-up visits at six month intervals. These six month follow up's cost $90 and must be paid in full by cash or credit at the time of your visit. Failure to keep your follow-up visits will result in the cancellation of your card.
Office hours for medical cannabis evaluations will be held on Saturday's from 8am - 12pm, beginning Saturday August 12th, 2017.

Certification for medicinal cannabis use is at the sole discretion of the provider evaluating you.
