Medication Request

For Routine Medication Request Refills

For routive refills, please contact your pharmacy and they will forward this request to us. We will respond to the pharmacy request within 24 hours. The pharmacy should then notify you when the prescription is ready.

For Medication Change Requests

Please complete our RX CHANGE REQUEST form to submit your request to your provider.

For Other Medication Requests

For a DOD refill, or a special medication request, please complete our OTHER RX REQUEST form.

Once your request has been placed, please allow time for the process to work, and we ask that you not call to check on this unless you have not heard from anyone within 48 hours. Repeated calls actually slow down the process. Your pharmacy will notify you when the RX is ready to be picked up.

URGENT CALLS

All urgent calls during closed office hours can be directed to the Hospital Operator and the on-call provider can be called. If the phones are not working, you can call the Hospital Operator at 443-481-1000 and they will page your on-call provider.
