Cold, Flu, COVID, Strep, or Allergies? How to Tell the Difference This Fall

With the seasonal shift comes sniffles, sneezes, and coughs — but not all symptoms mean the same thing. As a provider, one of the most common questions I get this time of year is some version of, “Is this just allergies, or am I actually getting sick?” It’s a great question, because how you treat it — and how quickly you should be seen — really does depend on the answer.

Cold, flu, COVID, strep, and allergy symptoms can look similar on the surface — congestion, coughing, sore throat, feeling run-down — but they have very different causes, timelines, and treatments. Knowing which one you’re dealing with helps you manage symptoms appropriately, avoid unnecessary worry, and know when it’s actually time to come in rather than wait it out.

Here’s how to tell them apart, and what to do about each:

Common Cold

A cold typically starts gradually rather than hitting all at once. You’ll usually notice a runny or stuffy nose, sore throat, mild cough, and a bit of fatigue. Fever is uncommon with a standard cold — if you’re running a high fever, it’s more likely something else.

What helps: Rest, fluids, over-the-counter cold medications, and nasal saline are usually enough to get you through. Give us a call if symptoms last longer than 10 days or worsen after 5-7 days.

Seasonal Allergies

Unlike a cold, allergies aren’t caused by a virus at all — they’re your immune system reacting to triggers like ragweed, mold, and pollen, all of which are in full swing this time of year. Look for frequent sneezing, itchy eyes, nose, or throat, congestion, and clear (not colored) nasal drainage. Allergy symptoms also tend to stick around for weeks, not days.

What helps: Antihistamines, nasal steroid sprays, limiting outdoor exposure on high-allergen days, closing your windows, and using air purifiers indoors can make a big difference. If you deal with allergies every fall, don’t wait until you’re miserable — an allergy evaluation can help us build a plan that gets ahead of your symptoms next time.

Influenza (Flu)

The flu is a different animal altogether. It tends to come on suddenly and hit much harder than a cold — think high fever, body aches, significant fatigue, headache, and a dry cough that seems to show up out of nowhere.

What helps: Rest and hydration are still key, along with fever reducers to manage symptoms. Antiviral medications can also help shorten the illness and reduce severity, but they only work if started early. Timing really matters with the flu — if you suspect it, call us and get tested as soon as symptoms start rather than waiting to see how you feel in a few days.

COVID-19

COVID can actually look a lot like the flu, which is exactly why it’s so hard to tell apart without testing. Common symptoms include fever, cough, fatigue, body aches, sore throat and headache, but COVID may also come with a loss of taste or smell and can range from very mild to quite severe depending on the person.

What helps: Rest, fluids, and monitoring your symptoms closely — especially for any trouble breathing. Testing is really the only reliable way to know for sure whether you’re dealing with COVID versus the flu, so if you’re not feeling well, it’s worth getting tested rather than guessing. Timing really matters with the COVID-19 — if you suspect it, call us and get tested as soon as symptoms start rather than waiting to see how you feel in a few days.

Strep Throat

Unlike the others on this list, strep is caused by bacteria, not a virus — and it comes with a telltale symptom: a sore throat that hits hard and fast, often without the runny nose or cough you’d expect from a cold. Look for a sore throat, pain when swallowing, fever, headache, lymph node enlargement, and sometimes white patches on the tonsils.

What helps: Strep needs to be confirmed with a rapid test or throat culture, and if positive, it’s treated with antibiotics. If you or your child has a sudden sore throat without other cold symptoms, it’s worth scheduling an appointment.
